Please note the following:
- If you decide to reactivate your membership in the future (and we hope you do), please note that the membership rates in effect at the time of reactivation will apply, as membership rates are subject to change.
- If you’re sure you’d like to cancel, please complete and submit the form below. This will serve as your 4-week written cancellation notice as required by your membership agreement. Your membership will be canceled 4 weeks from the submission of this form. Please note that if you have a scheduled renewal payment within these 4 weeks, the payment will be processed as scheduled. All payments are non-refundable.
- Cancellation of membership before the expiration of any specified commitment period requires a termination fee equivalent to the balance owed for your time with an active membership. For example, if the difference between a 26-week agreement and a month-to-month agreement is $15, and the 26-week agreement is terminated early, you will owe the balance for all active months upon cancellation.
